You were automatically billed for LSU's medical health insurance for this Summer 2012 semester if you enrolled this Spring 2012 semester and did not provide proof of private medical health insurance approved by International Services (I.S.)   You will not be billed for Summer 2012 insurance since your LSU's medical health insurance coverage expires August 13, 2012.   If your dependents were added by our office to your Spring 2012 fee bill, your dependents are also covered under this medical health insurance through August 13, 2012.

PLEASE NOTE:

• Currently enrolled F visa status holders are NOT eligible to upgrade or downgrade their insurance plan during the 2011-2012 policy year.  You will be able to upgrade/downgrade/change your plan at the beginning of the Fall 2012 semester.   
• If you have a private insurance carrier policy, please click on this link for important information Private Option. You are required to submit an updated “Insurance Coverage Evaluation Form” at the beginning of each and every semester regardless of the end date stated on any previously submitted forms.
