J-1 Student visa status holders

J-1 students and their J-2 dependents are required to have insurance which meets all mandatory requirements set forth by the United States Department of State and the University.

  • J students may only enroll in the BASIC PLAN based on U.S. Department of State federal regulations.
  • J students MUST enroll their J-2 dependent(s) in their BASIC PLAN or in a private insurance plan which meets all U.S. Department of State federal regulations and University requirements.  If you added your dependents to your Spring/Summer 2012 plan, you do not need to add your dependent(s) again this summer 2012 semester.
  • J students enrolling their dependent(s) in their BASIC PLAN MUST submit the "Summer 2012 Insurance Request Form" to International Services, 101 Hatcher Hall, by Tuesday, June 12, 2012 (absolute deadline date No exceptions will be considered. Form is located at the front desk of  International Services, 101 Hatcher Hall,  or may be downloaded here.
  • J students with private medical health insurance plans, please open this link: Private Medical Health Insurance Option
  • It is your responsibility to check your fee bill and/or to inquire with International Services at the beginning of the Summer 2012 semester to determine if your submitted form was received.
