MEDICAL HEALTH INSURANCE REQUIREMENTS:

 

Under LSU Student Accident & Sickness Insurance Plan (SASIP), new students with F and J visa status will be automatically charged $273 for the BASIC PLAN and the Repatriation/Medical Evacuation fee for the Summer 2011 semester. Approval and waiver of the automatic insurance charge is contingent upon the student’s health insurance coverage fulfilling all the requirements listed on the "Insurance Coverage Evaluation Form" (also listed below under Medical Health Insurance Requirements).  Copies of insurance cards and/or policy brochures/statements are not accepted as proof of health insurance coverage.

  • For multiple party plans, maximum deductible is $500 per person, not to exceed $1000.
  • Emergency and non-emergency care (No emergency only policies will be accepted.)
  • Maternity benefits coverage (regardless of gender) of 36 weeks gestation, labor and delivery. Please refer to the following website for sections 1681 and 1687 of Title IX of 1972 regarding equal treatment of male and female students: http://www.dol.gov/oasam/regs/statutes/titleix.htm
  • Dates of health insurance coverage must cover the student at least from the first day of classes to the last day of final exams for the semester in which the health insurance waiver is requested.  For the Summer 2012 semester, the dates of coverage must begin on or before June 4, 2012 and end on or after July 26, 2012.
  • If your private insurance policy covers all mandatory requirements with the exception of repatriation and medical evacuation benefits, you may present your "Insurance Coverage Evaluation Form" to International Services and you will be charged $3.00 for repatriation/medical evacuation benefits for the Summer 2012 semester.

PLEASE NOTE:  The "Insurance Coverage Evaluation Form" requires your insurance provider to indicate "Yes" or "No" for coverages on each of these requirements.  If any of these required coverages are marked "No", except for repatriation and/or medical evacuation benefits, please be aware that the LSU health insurance charge will not be waived.
