Upgrade From Basic Plan To Enhanced Plan

ONLY NEW INCOMING students this summer 2012 semester who hold F visa status are eligible to upgrade their plan from the BASIC PLAN to the ENHANCED PLAN by filling out the "Summer 2012 Insurance Request Form" for the period June 5, 2012 to August 13, 2012.  This form is located at the front desk of International Services in 101 Hatcher Hall or can be downloaded from here.  The additional charge for the upgraded plan will be added to students’ accounts AFTER registration is completed.

The deadline date to submit this form to International Services is Tuesday, June 12, 2012 (absolute deadline date.  No exceptions will be considered.)

PLEASE NOTE:

• J students and their J-2 dependents may only enroll in the BASIC PLAN based on U.S. Department of State federal regulations. 
• It is your responsibility to check your fee bill and/or to inquire with International Services at the beginning of the Summer 2012 semester to determine if your submitted form was received.
